Since 1977, Community Healthlink has been helping adults, children, and families recover from the effects of mental illness, substance use, and homelessness, with unique and unparalleled access.

Our “Front Door” model provides three ways for individuals to access our services:

We provide 24-hour confidential care - help without the wait.

Those experiencing a crisis or urgent needs are connected with services within 24 hours, and for non-urgent needs, within 10 days of initially contacting CHL.

Community Healthlink provides the following core services:
  • Person-centered treatment planning

  • Screening, assessment, and diagnosis

  • Outpatient behavioral health services

  • Crisis services

  • Outpatient primary care screening and monitoring
  • Targeted case management

  • Psychiatric rehabilitation

  • Peer and family supports

  • Intensive community-based outpatient behavioral health care for members of the US Armed Forces and veterans
In addition, CHL provides or will connect individuals with the following support services:
  • Primary care

  • Employment services

  • Family services

  • Child care

  • Transportation

  • Education services
  • Housing support

  • Social/recreational activities

  • Consumer-operated services

  • HIV testing
